Scientists document and test new continuity of care

Continuity of care is essential to the clinician/patient relationship within family medicine. However, researchers have few ways to track continuity of care as a measurement of clinical quality. Researchers developed the Physician Level Continuity of Care Measure (or CoC measure for short), which only recently received full National Quality Forum (NQF) endorsement.

Dr. Bauman sits down with Dr. Phillips to discuss his work with the National Academies of Sciences, Engineering, and Medicine’s report on Implementing High-Quality Primary Care. Dr. Phillips takes us through the development of the report, as well as the importance of a contextual, person-centered approach to primary care that focuses on the relationships with patients, families, and communities.
