The Continuity of Care Measure

Physician-Patient relationships are a hallmark of primary care, and physicians should be rewarded for maintaining them over time. That’s what ABFM’s Continuity of Care measure is deigned to do.

Why Continuity of Care?

  1. Improved patient outcomes and physician well-being.
  2. Decreased health care costs including total costs, ED costs, inpatient costs, primary care costs, and costs for specific conditions or treatments.
  3. Decreased health care utilization such as ED visits and hospitalizations.
  4. Critical for addressing equity and social risk.
  • Nationally endorsed

  • Available for all private health plans to adopt as part of the Core Quality Measure Collaborative (CQMC) core measure set.

  • Available in the PRIME Registry for all primary care clinicians and for use in Medicare’s Merit-Based Incentive Payment System (MIPS) Program.
