The Continuity of Care Measure
Physician-Patient relationships are a hallmark of primary care, and physicians should be rewarded for maintaining them over time. That’s what ABFM’s Continuity of Care measure is deigned to do.
Why Continuity of Care?
- Improved patient outcomes and physician well-being.
- Decreased health care costs including total costs, ED costs, inpatient costs, primary care costs, and costs for specific conditions or treatments.
- Decreased health care utilization such as ED visits and hospitalizations.
- Critical for addressing equity and social risk.
